Residential
Step 1: Order a qualifying Suddenlink TV offer through Smart Connection.
Step 2: Submit your final bill from your previous TV provider(s) showing the Early Termination Fee.
Step 3: Complete the online Contract Buyout Form.
Terms:
- Offer for new Suddenlink customers who are active customers of a competitor video service. Former Suddenlink accounts previously not in good standing or, who have disconnected service within past 30 days not eligible. Must provide copy of bill from competing video provider, in same name and at same address, showing early termination fee amount and that you were an active customer at time of sale without a past due balance.
- Bill may be submitted by email to [email protected] or by mail to Altice USA, Attn: DBB Program, 5110 80th St., Lubbock, TX 79424. Credit will be applied to account at the end of 60 days of services if all services are maintained and account is in good standing. Credit amount will match early termination fee shown on submitted bill up to max of $500 ($100 for Internet only customers).
Business
Step 1: Order a qualifying Suddenlink Business High Speed Internet + Business Class Phone offer through Smart Connection.
Step 2: Submit your final bill from your previous provider(s) showing the Early Termination Fee.
Step 3: Complete the online Contract Buyout Form.
Step 4: Send an email to [email protected] with the completed Contract Buyout Form and a copy of the final bill from your previous provider showing early termination fees.
Terms
- Once Suddenlink has verified your eligibility, credit will be applied to your account after 90 days of consecutive service that is equal to the early termination fee charged by your previous provider.
- Customer must subscribe to Internet and Phone and maintain all applicable services through promotional period to receive the stated price.
- Available only to new qualifying customers who have not subscribed to Suddenlink services within the previous 30 days and have no outstanding obligations to Suddenlink.
- Customers must provide a copy of their statement indicating the early termination fee, in the same name and at the same address, in order to qualify for the one-time buyout credit.
- Forms must be received within 60 days of installation of the Suddenlink data and phone.
- Credit amount will be determined by the early termination fee on the final bill from the previous provider, not to exceed $500.
- Final bill for services rendered by competition does not quality for buyout credit under this program. Credit will be applied to account at the end of 90 days of services if eligibility of both phone and data services are active and account is in good standing. Price excludes taxes, fees and surcharges. Three-year contract required and subject to early termination fees. Offer subject to change. Other restrictions apply.
